Care Plan
Each care plan is developed considering the clients' individual needs. We can involve the client's family in this initial visit and throughout the care planning process, should the client do so wish.
The client's care plan is developed following the initial needs assessment completed by one of our experienced care managers and any information gathered is held in complete confidentiality.

Our care plans will document all agreed times and comprehensively explain all agreed tasks that will be undertaken by our fully trained care staff along with specific times agreed. A copy of the care plan including all risk assessments, along with a service user guide will be made available for each client.
Each client will also receive an information pack with other useful contact information along with complaints procedures should they wish to raise a concern.
Risk assessments are carried out by one of our qualified members of management , our risk assessments are completed to identify any potential risks that may occur during a care visit , to either the carer or the client. We also have an electronic monitoring system on which the carer will document a summary of their care on every visit, which will give an overview of their care and any interventions that occurred during the care visit.
